University Archives Permission Request Form

Requests to use items from MC’s Archives should be submitted through this form to the Office of Marketing and Communications. The submitted form will be reviewed by the Office of Marketing and Communications and The Edward L. McMillan University Archives.

Requests should be submitted at least 14 business days in advance of the requested use date.

Permission to use and/or publish is granted only upon completion of the application and execution by authorized representatives of both parties. I (Applicant) agree to abide by the terms and conditions listed in this Agreement. Permission is granted for one-time, non-exclusive use only. Materials must not be altered beyond standard formatting (e.g., minimal cropping for layout), and credit must be given as specified. Any reuse requires a new request. By completing and submitting this form, I (Applicant) agree to comply with the stated terms and to properly credit all materials used. I (Applicant) understand that I am responsible for securing copyright or other intellectual property clearance if the archive does not hold those rights. I (Applicant) release the archive and the university from liability for any copyright or other intellectual property infringement.
